The Roosevelt High School Health Center is located on the second floor of Roosevelt High School in Room 255.

FROM I-5 NORTHBOUND
Take Exit 170 - Ravenna Boulevard off of the freeway. Stay straight at bottom of ramp to go onto 8th Avenue NE. Turn right onto NE 63rd Street. Turn left onto 14th Avenue NE. Roosevelt High School is at the intersection of 14th Avenue NE and NE 65th Street

FROM I-5 SOUTHBOUND
Take exit 171 - NE 71st/NE 65th Street off the freeway. Merge onto 6th Avenue NE. Take a slight left onto NE 70th Street. Turn right onto Roosevelt Way NE. Turn left onto NE 66th Street and follow until you reach 14th Avenue NE. Roosevelt High School is at the intersection of 14th Avenue NE and NE 65th Street.

Appointments

Before or after school appointments are available. Request to Report slips are given for appointments during school. We will do everything possible to schedule or reschedule so students miss as little class time as possible. Students may also visit the clinic without an appointment by getting a pass from their teacher or when referred by the school nurse.

When the school-based health centers are closed, students are encouraged to call Neighborcare Health's 45th Street Medical Clinic at 206-633-3350. Dental appointments are available at 45th Street Dental Clinic at 206-548-2964.

What to Bring

Students must be registered with the Roosevelt High School Health Center in order to receive care in the Center. A completed registration form with parent or guardian signature is all that is needed to access all of the services offered. Students 18 years old and over can sign registration forms themselves. Students under 18 can register themselves for confidential care, including reproductive health services and mental health counseling.

Fees and Insurance

No student will be denied services due to inability to pay. Because the Families and Education Levy covers only a portion of the cost to operate Roosevelt High School Health Center, we bill insurance companies when possible.

For students who have coverage through medical coupons or a public insurance plan, we bill all services to Medicaid, and the student/family will have no out-of-pocket expenses.

For students with private insurance, it is possible that parents/guardians may have some out of pocket cost, depending the type of services and on the private insurance company’s coverage. Not all services are subject to insurance company policies, so please call if you would like more information. Lab services for non-confidential care are billed to the student’s private insurance by Lab Corp.

Neighborcare Health can assist uninsured students and their families with Medicaid enrollment and have staff that can help with the necessary paperwork.

As a patient of Neighborcare Health, you have a responsibility to:
Provide complete information about your health history and current health.
Participate in decisions about your health care treatments and procedures.
Follow the treatment plan developed with your provider or inform your provider if you cannot follow through.
Treat Neighborcare Health staff, patients and property respectfully.
Make and keep appointments and notify the clinic if you are going to be late or need to cancel an appointment.
Pay the bills and fees for the health care services provided to you.
